Health Care Proxy Form:


A legal document that allows one to appoint a health care agent.  A person to make health care decisions in the event one is not able to make decisions for ones self.

One may restrict the ability of this person or allow them to make all health care decisions.

An attorney is not required to draft these forms and most states only require the signature of two witnesses to make the form legal.
